Institution-wide WAC surveys

Hi:

I am currently teaching in an institution in which a massive assessment
project is underway.  One aspect of the project involves WAC and the way
the policy is being applied in the disciplines at Delta.

Previously, I've seen a number of different faculty surveys that gathered
info about what types of writing were being assigned in the various
disciplines.  Do any of you out there have samples of surveys you've used
in your institutions, either as writing center research instruments or WAC
program research instruments?
