RE: data collection forms

Hi all,

I have been reading this thread....mostly at home and for many 
reasons I can't post using this e-mail address from home.  In any 
case that URL is technically correct, but we had a recent 
switchover of our web services here, and I need to find a way of 
getting the archive back up and online.  I will let everyone know 
when that has been accomplished.
