Re: data collection forms



Rebecca, attached (in Word format) is a copy of our form, plus the FAQs that we
print on the reverse side of the writer's copy.

Our form is printed on 3-part NCR paper; the original is given to the student
immediately after the consultation, the yellow (2nd) copy is kept in the UWC,
and the pink (3rd) copy is sent to the professor, unless the student requests
otherwise.  We send an explanatory note to the prof along with any pink forms (I
attached that too).  All students who work here sign a confidentiality
agreement--a form used by all univ. offices which deal with student records.

If a student asks us not to send a copy of the form to the professor, and the
professor calls me later to ask if the student used the UWC, I will say yes or
no--but no details. (Campus attorneys advised me that this is about as
confidential as we can be.)  No professor has ever called me with that request,
fortunately.

We collect similar information via our appointment scheduler (linked from our
website at http://reach.ucf.edu/~uwc).  Writers tell us what they're working on,
etc., when they make an appointment.  We verify that info during the
consultation, and writing consultants make any corrections/additions to the
database as they have time, using the paper copy of the Record of Consultation
form.
