We use FileMaker Pro in our center. Once you customize the database to fit
your needs, you can search by any combination of data--it is wonderful.

The problem is getting students to give you all of the needed information. 
We used sign-in slips in the past, sometimes they work and sometimes they don't.
We have since stopped using the slips because we are funded by a technology fee
each student pays each semester and do not need to "justify" anymore.
