request for survey instruments

As a complete newbie to the list, I've enjoyed the collegial tone of the
festivities re Byron Stay's birthday and garnered a great new recipe in the
process.  Off to a rollicking start.  Now a plea for help--and an apology in
advance if this is old territory I'm about to cover:

Our writing center task force is working on a self-study and is in the
process of designing a survey to distribute to our English faculty (with 70+
faculty members, that's a lot of potential data!).  We aim to tap into their
perceptions of our writing center, their suggestions for improving our level
of service to their students, and any other useful information we can glean
from such a survey.  Later, we'll extend our study to include faculty
campus-wide.

Does anyone out there have a sample survey they wouldn't mind sharing with
us as a model, plus any advice/words of wisdom from those who have done this
before?  Additionally, we'd eventually like to compare our results to those
of other centers, so we'd love to hear from any of you who will share your
own results of such a survey.
