Barbara, that's a great idea. Besides seconding the suggestion of getting the 
professor's approval, let me add one other suggestion based on what a good 
friend of mine (a copyright lawyer) told me when I started compiling student 
papers for my research: get the student's HOME address and social security # on 
the form. If for some reason you need to track the student down, and she or he 
has left campus, this makes your life much easier. The SS# helps if you have 
enrolled (as we currently do), three Claudia Morales-es, two of whom have middle 
initial M.
