permission forms



Greetings!  We wc people at FMU are trying to create a file of annotated 
student papers in the center (successful papers complete with instructors' 
comments and grades) to use during tutorials.  Currently we are developing a 
form on which students will give us permission to use their papers for use in 
the center, research, etc.  So far the form will include the following:

- title of paper
- student's name
- instructor's name
- place for student to check either "I do" or "I do not" give permission for 
my paper to be used in the Writing Center, professional research
- place for student to check either "I do" or "I do not" want my name on the 
paper 
- date
- student's printed name and signature

What else might we be forgetting?  Maybe the option of leaving the 
grade off?  And getting the *professor's* permission as well suddenly comes 
to mind...

Any thoughts on how to be responsible, helpful and ethical all at once?
