Ours center works on drop-in (mostly) and appointments (sometimes).  At the
suggestion of the tutors themselves, we designed a simple form asking the
client to write out their interpretations of the assignment at hand and to
say a few words about whatever their professor had last emphasized in
classes.
        But no one seemed to use them.  I guess our tutors are formless.
