I agree with the Faceless bureaucrat very much--and I want to add that the
forms I have been talking about are for the most part filled in by the
tutors.  The students merely tell us their name, student id, class,
instructor, what they want to work on, and how they heard of us.  if they
are coming for the first time, they read and sign our ground rules.  At the
end of the session we encourage tutors to write a quick summary of the
session with the student, to encourgae closure.
