Neal reminds me that we also ask how students have heard of us, and we also
have a form for new people that is slightly different from the form for
returns.  The first form lists our "ground rules" and asks for a signatre.
The key itme is the one that asks for permission to use the records in
research (without using any names or SS#s).  Students may still get help if
they elect to omit that ground "rule," though no one seems to mind it.  We
keep these signatures on file in case we should want to use our records for
research (although that is really a very hypothetical situation still).
