I haven't done this recently, but the tutors did it at my last school. The
form only asked two questions besides the basic info (Name, date, teacher,
etc): What did the writer request of the tutor and what did the tutor do with
writer while he/she was there. There was no judgmental statement involved.
I found this particularly good on the secondary level because our tutors are
actually reader/listeners who ask questions of the writer. As director, it
was my job to read these and talk with the tutor to determine exactly what
occurred, how it could have worked better, what else we can do for the
writer, etc.
The big problems with all this are TIME, STAFFING, and TRAINING.
